Quatrefolic is an active form of folate at the cellular level, the form found in the circulation and the only one that crosses the blood-brain barrier. It may also be the optimal form to prevent neural tube defects. It does not need to be converted to the active form by the enzyme methylene tetrahydrofolate reductase (MTHFR), which is known to exhibit common single nucleotide polymorphisms. Administration of quatrefolic acid was associated with higher peak levels of (6S)-5-MTHF than folic acid and calcium 5-MTHF, even in individuals with high-functioning MTHFR polymorphisms.
Methylcobalamin is a cofactor for B12-dependent enzymes and has direct biological activity. Combined with other B vitamins, it reduces the risk of macular degeneration and migraines, with effectiveness sometimes modulated by MTHFR status.
